Bereavement Services


Hospice care does not end with the death of a patient. Hospice & Palliative Care Partners of Ohio provides family and friends with grief support and information for a period of one year following a patient's death. This follow-up program is designed to offer comfort, hope and guidance.

Rarely are families and friends fully prepared for the many strong feelings that follow the death of a loved one. Even when surrounded by others, there may be a feeling of being alone. It is not uncommon to have a sense that no one could possibly know what you are going through.

Bereavement Support Groups

HPCPO “Bereavement Services” include:

Our goal is to help families and loved ones understand their grief. These are the types of services offered during bereavement:

Visits and calls.
Following a condolence letter from the entire hospice team, families and loved ones will be contacted 4-9 weeks after the patient's death by either the hospice social worker or the bereavement coordinator, who will offer support and review the one-year bereavement follow-up program.

Articles and invitations.
Every few months you will receive articles that we hope you find helpful to you as you cope with your grief. While no two people grieve in the same way, it can be comforting or reassuring to read articles that focus on the very natural feelings and concerns that accompany the loss of a loved one. Quarterly hospice newsletters are another source of information.

Memorial and remembrance services.
HPCPO conducts memorial and remembrance services twice a year. We offer families the opportunity to come together to remember their loved one. All families are welcome to attend, whether their loved one was a recent patient or passed several years ago.

Bereavement support groups.
Support groups can be a safe place to work through grief issues. Families are sent information about current grief support groups. Many people find groups give them support and encouragement. There is no charge for support groups.

Support for children.
HPCPO recognizes that children often need special help dealing with grief and loss. Sometimes, adults are so overwhelmed with their own grief that they have difficulty comforting a child. We offer special activities throughout the year, as well as referrals to other community resources for these special situations. The hospice art therapist is also available to support the children.
