Hospice Care
Hospice & Palliative Care Partners of Ohio offers
nursing and physician services, counseling,
social services, spiritual support, crisis
intervention, and expressive therapy for
those facing a terminal illness. The HPCPO
team of professionals and volunteers work
together with patients, families, and physicians
to assure appropriate support and comfort.
Nursing and physician services
include:
- Assessment, diagnosis, and plan for treatment
of health problems
- Clinical management of pain and distressing
symptoms of illness
- Ongoing care coordination
- Education for patient and caregivers
- Clinical procedures and treatments
- Palliative care
