Palliative Care
The Palliative Care Consult Program provides advanced
practice nursing and clinical care for
patients and families who are facing
a life-limiting illness. Consultation assists
in the primary medical management of the
patient in collaboration with the patient's
attending physician.
Palliative care is the comprehensive management of a
patient’s physical, psychological, social,
spiritual and existential needs.
Services provided include:
- Assessment, diagnosis and
planning interventions for health problems
- Clinical management of pain and distressing symptoms of illness
- Referrals to community resources
- Ongoing care coordination and facilitation of services
- Education for patients and caregivers
- Access to off-hour support
- Clinical procedures and treatments
- Additional medical advice available
