Palliative Care

The Palliative Care Consult Program provides advanced practice nursing and clinical care for patients and families who are facing a life-limiting illness. Consultation assists in the primary medical management of the patient in collaboration with the patient's attending physician.

Palliative care is the comprehensive management of a patient’s physical, psychological, social, spiritual and existential needs.

Services provided include:

• Assessment, diagnosis and planning interventions for health problems
• Clinical management of pain and distressing symptoms of illness
• Referrals to community resources
• Ongoing care coordination and facilitation of services
• Education for patients and caregivers
• Access to off-hour support
• Clinical procedures and treatments
• Additional medical advice available
