Payment/Eligibility/Reimbursement
Payment
Hospice & Palliative Care Partners of Ohio
provides hospice services under private
insurance, the Medicare Hospice Benefit
and the Medicaid Hospice Benefit. Non-insurance/self- pay
are accepted and subsidized as funding
permits.
Eligibility
HPCPO care is made available when:
- Care is approved by a physician
- Patient desires hospice
care
- Patient has a life
expectancy
of only months
- Treatment offered is palliative in nature
rather than curative
Reimbursement Policy
- HPCPO accepts assignments for all Medicare
and Medicaid patients
- HPCPO will bill all private
insurance carriers when applicable
– Aetna,
Anthem, Medical Mutual, etc.
- All claims submitted to insurance carriers are subject to medical review
- If your private insurance carrier does not reimburse our services entirely
(i.e. 80%, etc.), you will be sent a bill for the balance due. Your insurance
company should supply an explanation of benefits directly to you.
- If you have no insurance coverage, our business office will contact you
to discuss reimbursement of our services. You may be asked to complete
a financial data form to assist us in setting appropriate fees for our services.
