Pre-Hospice Care

The pre-hospice program offers skilled services for patients facing a life-threatening illness. The pre-hospice staff is trained in all aspects of end-of-life care, including pain assessment and symptom management. We are knowledgeable in disease progression and can assist patients and families with the decision-making process as their condition changes. Through shared staff, the program offers a smooth transition from pre-hospice care to the certified hospice program.

• Pain and symptom control
• Disease management
• 24-hour crisis support
• Home care aides providing in-home personal care assistance
• Education and instruction for patient and family caregivers
• Multi-disciplinary care can include MSW, PT, OT, ST, HCA
• Collaboration with the palliative care consultation team
