What are the criteria for Hospice Eligibility?

  • Life-limiting disease and life expectancy of months or weeks to live
  • Services approved by a physician
  • No requirement for skilled nursing or homebound status
  • No longer a need to pursue aggressive treatment
  • Resident of the following northern Ohio counties: Cuyahoga, Lorain, Medina, Summit, Lake, Geauga, or Portage

What information is needed for a referral?

  • Patient's name, address, phone number, medical diagnosis and insurance provider
  • Physician name and contact information
  • Patient's primary contact or caregiver

What are included in the Hospice Team Services?

  • Physician and nursing services
  • Social workers
  • Home care aides
  • Bereavement services
  • Spiritual care volunteers
  • Art/massage therapy
  • Rehabilitation
  • Inpatient care
  • Respite care
  • 24-hour on-call services
  • Financial coverage of medicines and supplies for terminal illness
